Pre-Award Forms

SciENcv

Faculty submitting to NSF can use SciENcv: Science Experts Network Curriculum Vitae as an NSF-approved format for use in preparation of the biographical sketch and Current/Pending sections of an NSF proposal. Faculty can create a profile that include all pertinent information needed for their documents and SciENcv will produce an NSF-compliant PDF version of the biographical sketch and Current/Pending forms that can then be given to your pre-award specialist for upload. A bonus of using this method is that a SciENcv account profile can be used to create an NSF and an NIH biosketch.

Subaward
Inclusion Form

Principal Investigators (PIs) including subrecipients in their proposals must complete this form and return it to their Pre-award Specialist before submission.

Subrecipient
Commitment Form

All subrecipients must complete this form when submitting a proposal to KSU, along with documents and certifications required by sponsors. This form must be endorsed by the Subrecipient’s Authorized Official.

Cost Share
Authorization Form

Cost sharing is a commitment of real dollars to a sponsored poject. All proposals that include a dollar amount for cost share, whether as part of the budget or listed in the proposal narrative, must have a signed cost share form on file with Sponsored Programs at the time of proposal submission.
